Canceling an Appointment
To cancel your appointment log into the Patient Portal Gateway
Cancellation Policy
- All Appointments Are Subject to a Cancellation Fee if Not Cancelled at Least 24 Hours in Advance.
- Any Appointments Cancelled Within the 24-hour Window Are Subject to the $25.00 Late Cancellation Fee.
- Missed Appointment Fee: $55.00.
