  • Log on to Gateway
  • Select Messages
  • Select New Message
  • Select "Tuberculosis (TB) Request"
  • Complete the selected questionnaire and submit

Complete the selected questionnaire and allow at least 3 business days to receive a response with further instructions.

You will be notified by Student Health if additional items are required for clearance. If you have a TB test requirement, you must have a TB test that is dated within one year of entry at UCSB.

General Information

  1. You must complete the tuberculosis risk screening process and enter your vaccine information information prior to registering for classes. There might be a hold on your registration until you have complete ALL steps of this process.
  2. Carefully read and follow the instructions on the TB Health Assessment Form. Incomplete forms will not be accepted and you will NOT be cleared.
  3. The TB clearance process begins by completing a tuberculosis screening questionnaire in the Student Health Patient Portal, fax or email.
  4. If your TB risk screening shows you to have no risk factors for TB, you should expect to receive a secure message within 7 business days confirming you are compliant with this requirement.
  5. If your TB risk screening shows you to be a higher risk, you will need to have further testing completed.
    1. Download the TB health assessment form, take it to your Primary Care Provider (PCP).
    2. Once your PCP has determined you are free of active TB by conducting any additional testing, they must complete and sign the TB health assessment form. The completed and signed form must be returned online, by fax or email.
    3. Confirmation of receipt of document(s) is not possible.
