Medical exemptions
Medical exemptions to the UC Immunization Policy are only allowed for conditions established by the CDC, or the Advisory Committee on Immunization Practices (ACIP) and must be submitted for individual shots by in a written statement from a licensed medical physician or nurse practitioner (M.D., D.O., P.A. or N.P.) which states:
- That the physical condition or medical circumstances of the student are such that the required immunization(s) is not indicated.
- Which vaccines are being exempted.
- Whether the medical exemption is permanent or temporary.
- The expiration date, if the exemption is temporary.
Vaccination Medical Exemption Form
Please submit the Vaccination Medical Exemption Form by email or fax to 888-972-9735. You may also upload your form when you complete the "Required Immunization Records" as part of your Steps to Enrollment.
Your correspondence needs to include the following:
- Full name
- Date of birth
- Perm number

Exceptions for religious beliefs are permitted per the University of California, Office of the President.
