  1. Please list past and present college co-curricular involvement.
    Include date of service and extent of involvement.


  2. Three important skills that the Patient Advocate possesses are good listening, problem solving and being
    a team player. Briefly describe how each of those skills pertain to you and the situations you've used them in.


  3. Which of your experiences at UCSB have been factors in the development and enhancement of your
    skills and abilities?


  4. This position involves a lot of public speaking--sometimes in small committees, other times in large
    lecture halls in front of hundreds of students and/or parents. This is a two-part question.
