Itemized Billing Statements are available through the "Gateway", our secure browser.
After you sign on, click on the left hand side under "Billing Statements". You will be able to print our your charges and use it to submit to insurance companies.
Students can also download an Itemized Billing Request Form. This form can be:

BROUGHT into Student Health:
Dropped off at the information kiosk in the lobby.
Request forms are also available in the lobby. All requests will be processed within 1 week and mailed to the address that you provide on the form.
