Important health information for EAP students & others planning International Travel
We are having difficulties with student access to EAP forms in "The Gateway". If you are having problems, please come into Student Health to access forms in person.
If you are planning to study abroad, participate in research, or travel to a developing country, you are encouraged to utilize our travel clinic services. A pre-travel appointment at the travel clinic should be made 6-8 weeks prior to departing. Several vaccines need to be given at intervals in order to avoid adverse interactions and to provide adequate protection. During times of peak demand it may take up to 4 weeks to get an appointment with the Travel Clinic.
NOTE:
- There is a fee for the visits and immunizations.
- When you schedule your appointment, please inform our staff of every country that you are planning to visit and be certain to bring your immunization records to your appointment.
Services offered to travelers include:
- Individualized consultations with a travel nurse or physician for information regarding foreign travel including current recommendations from the Centers of Disease Control and International Society of Travel Medicine.
- All recommended and required immunizations and International Certificates of Vaccination
- Prescriptions and information for prevention of malaria and other illnesses
- Up-to-date, country specific health and travel advisories
Important Information for Your Appointment
- Call (805) 893-3559 to schedule your appointment at least six to eight weeks before departure.
- Bring your immunization record and/or International Certificate of Vaccination ("the yellow card") with you.
- Be prepared to discuss your full itinerary and planned activities.
- If you are planning to travel to certain countries with EAP (see below)*, you must take the online travel course in order to receive your required travel certificate. Please follow the instructions and complete the course prior to your travel appointment.
*ONLINE TRAVEL HEALTH EDUCATION CERTIFICATION COURSE IS REQUIRED FOR STUDENTS TRAVELING TO THE FOLLOWING COUNTRIES WITH EAP:

INSTRUCTIONS:
- Go to https://myeap.eap.ucop.edu/travelhealth and follow the instructions for the course.
- Please allow 1 hour to complete the course. IMPORTANT NOTE: Do not exit the course at any point before its completion. You cannot save your work and exiting will cause you to have to start the course over from the beginning.
- You should have access to a printer in order to print 3 copies of your certificate at the end of the course.
- Mail one copy to UOEAP (address is on the certificate) by the designated deadline
- Bring one copy to your appointment with your travel health provider
- Save one copy for your records
After you complete the online Travel Health Course, you must make a Travel Appointment with a travel health provider (at Student Health, your private physician*, or a local travel clinic*) as soon as possible. The purpose of this required appointment is to discuss (and receive, if indicated) immunizations for vaccine-preventable diseases you may be exposed to during travel. Also, if you will be traveling to a malaria-endemic region, you will need an appointment to discuss malaria chemoprophylaxsis.
To make a Student Health Travel Clinic appointment:
- Call the Travel Appointment line at 805-893-3559. Please leave a message with:
- Your full name
- Perm #
- Phone # (including area code)
- Destination country and program name
- Best days and times for an appointment
- You will receive a return phone call with appointment date and time
- You will receive a U-mail reminder 2 days before the appointment
- This appointment CANNOT be booked online.
*Students going to the following countries cannot see a private physician or local travel clinic and must be seen in Student Health: Botswana, Ghana, India, Senegal, Tanzania.
