Student Medical Checklist

Before you come to UCSB, there are a number of ways you can plan ahead to avoid or minimize the complications caused by illness. 

Request What to Do
First-aid kit Bring a first aid kit, including a thermometer and over-the-counter medications. If you forget, our pharmacy has these items.
Remember, once on campus, UCSB Student Health is available to all students, regardless of health insurance coverage.
Health insurance coverage If you are covered under a health insurance plan other than the University of California Student Health Insurance Plan (UC SHIP), bring a copy of your health card (front and back) to campus. It is important to check your policy to be sure you are covered  “out of area” for non-emergencies.
Health summary Bring a health summary from your health care provider or have your medical records faxed to (805) 893-2758 if you have chronic health problems (such as asthma or diabetes). This helps Student Health staff provide you with the best possible care. We also suggest you make an appointment to establish care with a provider before you become ill if you have ongoing health needs. Call (805) 893-3371 for an appointment or go online after you arrive on campus.

You may want to ask your family to help you fill out the Health History Form that Student Health needs on your first visit.
Hepatitis B
immunization series
Series of three doses required for enrollment at UC (for those under 19 on first day of classes) unless there is a medical contraindication. Hepatitis B immunizations are available. See Hepatitis B for more information.
Immunization records We strongly recommend students to send their immunization records to Student Health at SHSImmunizationRecords@sa.ucsb.edu or fax them to 805-893-2758.
Immunization recommendations In addition to the Hepatitis B immunization requirement, Student Health also recommends the following vaccines found on the Immunization Checklist of Recommended Shots.

Prescription
medications
Bring enough of your ongoing medications for the first month of school, and bring a copy of the prescription with you or have your medical provider call it directly to our pharmacy at (805) 893-2116. Plan ahead for refills. Student Health Pharmacy can fill your prescriptions and also has other low-cost health items for purchase.
