Insurance ID Card

You do not need to have your UC SHIP card to be seen at student health  however, you will need your UC SHIP ID card to obtain services outside of Student Health, such as when you are referred to outside specialty providers or for specialized imaging, or if you need to visit a hospital or emergency department. Your UC SHIP ID card contains unique numbers that apply to both your medical/vision coverage (Anthem Blue Cross) and dental coverage (Delta Dental). We advise that you have your UC SHIP ID available at all times, and especially if you are travelling.

ID cards for medical and vision coverage can be generated anytime using your UCSB Net ID. A hard copy will also be sent to the local address you have listed in the Registrar’s GOLD system, after the third week of classes in the initial quarter of the academic year you enroll. If you did not provide a local address, the card will be sent to your permanent one.  You can also call the customer service line at Anthem Blue Cross (866) 940-8306 to order replacement cards. The UCSB Student Health Insurance Office can also print out UC SHIP ID cards in emergency situations, and is open during regular working hours without appointments needed, or by calling (805) 893-2592.

ID cards for dental coverage are generally not required to obtain dental care, but a hard copy will be mailed to you after the third week of classes in the initial quarter of the academic year you enroll. Your ID number for Delta Dental coverage is the same as the number on the UC SHIP ID card from Anthem Blue Cross. If you wish to obtain a separate Delta Dental card, you may print one out from their website https://www.deltadentalins.com/ or call or the customer service line at Delta Dental (800) 765-6003 to order replacement cards.
