Itemized Billing
Students who don’t have USHIP, and would like to request their private insurance company reimburse them for Student Health charges; can download a
Itemized Billing Request. This form can be either mailed to us at the address on the form or brought into Student Health and dropped off at the information kiosk in the lobby. All requests will be processed within 1 week and mailed to the address you fill in on the form.
