	Clinical Services Auxiliary Services Eye Care Appointment based - fee-for-service; complete vision care (i.e. corrective lenses, frames, contacts, etc.). Dental Care Appointment based - fee-for-service; complete general dentistry (i.e. cleanings, exams by dentist, fillings). Physical Therapy Appointment based - physician referral required; rehabilitation after injury or surgery. Pharmacy Prescription and over-the-counter medications. Laboratory Processing fee charged per day (not per test); most tests are analyzed in-house however, some tests are sent to a reference lab.
